Release of Information Form & FAQs

In order for Alomere Health to process your request for the release of your medical records, you must complete a form called the Authorization to Release Protected Health Information. There are three ways to complete this form:

Fax & Mail

Print this form and fill out completely. Once completed, either fax the form to 320-762-6127 or mail the form to Alomere Health, Attention: Release of Information, 111-17th Avenue East, Alexandria, MN 56308. (Important note: Please complete the form in its entirety- we cannot process incomplete forms.)

Online

We partner with ScanStat to provide release of information services online.  If you would like to request records electronically, you can log on to www.scanstat.com.  Go to the upper right-hand corner and click on Patient Record Request and complete the request.

Release of Information Questions

If you have general questions on the authorization process, please email us at roi@alomerehealth.com or call 320-762-6086. Please do not add personal information through email (birthdate, dates of service, diagnosis, or procedures).

  • Is there a charge for a copy of my medical record?

    A patient’s first request for their own personal medical records is free; there may be a charge for additional records.

  • How long will it take to get a copy of my medical record?

    We usually process requests within five business days.

  • How can I obtain copies of my children’s medical records?

    Under most circumstances, a parent can authorize release and get copies of medical records for a child who is under age 18. The parent must complete and submit an Authorization to Release Protected Health Information form (available above).

  • Can I obtain copies of my spouse’s medical record?

    You can only obtain a copy with the patient’s signed authorization.

  • How do I obtain a copy of my birth certificate?

    We don’t have copies of birth certificates for babies born at Alomere Health. To obtain a copy of the birth certificate, you can call any County Recorder’s Office of Vital Records Division in the state of Minnesota, or contact the Minnesota Department of Health at www.health.state.mn.us.

Patient Satisfaction Survey

Please tell us about your experience.

The quality of your care and your overall experience at Alomere Health is of utmost importance to us. Please let us know what we did well and where we can improve by completing our patient satisfaction survey.

Background information on our patient survey:

We use a nationally standardized survey format called the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS). This survey was developed by the Center for Medicare and Medicaid Services (CMS) and it helps us know how we are doing here at Alomere, and in relation to other facilities nationwide.

The HCAHPS survey consists of 27 questions and covers your experience with:

  • Communication with doctors
  • Communication with nurses
  • Responsiveness of our hospital staff
  • Pain control
  • Communications about medicines
  • Cleanliness of our hospital environment
  • Quietness of our hospital environment
  • Discharge instructions

How does this survey get accomplished?

  1. An independent survey company randomly mails surveys to adult patients who were hospitalized in the medical, surgical, and OB units.
  2. The survey results are collected, recorded, and submitted to CMS.

Quality of Care Measures

Alomere Health is committed to providing high-quality care for all the healthcare needs of you and your family. The hospital is required to submit data to the Centers for Medicare and Medicaid Services (CMS) which reflect our performance in the following five areas: (a) heart attack, (b) heart failure, (c) pneumonia, (d) infection reporting, (e) surgical care, (f) stroke and (g) patient satisfaction (HCAHPS). This performance is called “quality of care” measures. The results of “quality of care” measures can be compared from hospital to hospital. As a patient or family member, this information is useful for you to learn about quality care and to become a more informed consumer to make the best healthcare decisions for you and your family.
